Globally, it is estimated that 2.3 billion people suffer from caries of permanent teeth. When a tooth hurts, most of the people think that removing the tooth is the most effective and the best way to deal with it.

Many people have a notion that root canal treatments are very painful, costly, and that after doing root canal treatment also, the pain will recur frequently and finally one day or the other the tooth has to be removed. So, its better to get it extracted on the very first day.

Question 1. – What is the structure of tooth?

Answer – A tooth consists of three parts –

  1. Outer hard and translucent layer is the enamel.
  2. Middle yellowish layer is the dentin.
  3. Innermost part comprises of the dental pulp which contains the blood vessels and nerve endings. It is the part of the tooth, which carries the sensations.

When a tooth hurts, it means the nerve endings in the pulp are affected.

Question 2. – What is a Root canal treatment?

Answer – In a root canal treatment, the infected nerves along with the pulp is removed, the root canals are cleaned, disinfected and filled with an inert material. It may require about 2- 4 appointments with a gap of few days to complete the root canal treatment. In some cases, single sitting root canal treatments can also be done.

Question 3. – Are root canal treatments really painful procedures?

Answer – Since a person needing a root canal is often already in extreme pain, the root canal treatment actually provides relief from pain and recovery time is very minimal. And when the pulp is dead, in that case also there is no pain.

Answer – The cost of root canal treatment varies, depending on the expertise of the performing dentist and the quality of material used by the dentist.

In spite of several measures taken to preserve a tooth, many times, a tooth may be missing due to decay (caries), gum disease, congenital absence of teeth (anodontia), injury or trauma, etc. Do you have a missing tooth? What are dental Implants?

Dental implants are mostly made of titanium and they act as a replacement for the roots of a missing tooth, on which the crown, bridge or removable dentures can be placed. 

What’s so special about dental implants?

The titanium metal of which the implant is made has a very special property. It can integrate (fuse) with the living jawbone, a process called Osseo-integration. That allows an implant to function very similar to the root of a natural tooth. And isn’t that what everyone wants from their teeth? 

Why should you consider replacing a missing tooth with dental implants?

  1. For aesthetics – Aesthetics is considered the main reason for which people consider replacing their missing teeth. Dental implants provide the best aesthetic results, as they mimic a natural tooth in form and function.
  1. For eating and restoring function – It becomes very difficult to bite and chew through a variety of foods if the teeth are missing. That can lead to difficulty in consuming proper diet and can give rise to deficiency of necessary vitamins and nutrients.   1. Oral examination of the patient – Oral examination of the patient is done to rule out any oral disorders like periodontal and gingival diseases.
  1. Evaluation of the implant site – A thorough evaluation of the implant site is carried out by clinical and radiographic examination of the implant site. With the help of 3D CBCT scans and other x-rays, the implant site is evaluated for bone quality and quantity.

Medications and soft diet is advised to the patient in the healing period of 6-7 days to relief the discomfort.

  1. Attaching the implant with the artificial prosthesis (crown/bridge/denture) – Depending on the planned prosthesis, a crown/bridge/denture is fitted onto the implant.

Limitations of dental implants –

  1. Implants may not be possible in all patients, like those with certain medical and oral disorders and with deficient jaw bone quality and quantity.
  2. Implant treatment may take longer than other treatment modalities, but as we know ‘Great things take time’, same is the thing with implants.
  3. Implants may cost more than other treatment modalities, but it gives long term results, while the life of a bridge/denture is far less.

Is there any special oral hygiene protocol to be followed after implant treatment?

Implants need no special oral care. Routine brushing and oral hygiene maintenance like what is recommended for natural teeth is all what an implant needs. It just acts like your normal teeth.

What is Oral Submucous Fibrosis (OSMF)?

OSMF is the condition of oral cavity, which leads to reduction in mouth opening due to fibrosis of the tissues in the oral cavity.

  • Progressive inability to open the mouth.
  • Reduced movement of the tongue.
  • Burning sensation in the oral cavity on consumption of spicy food.

Causes of OSMF?

  1. Betel-nut (supari) chewing – Most important

  5. Nutritional deficiencies

What are the grades of OSMF?

OSMF is classified into 4 grades based on the inter-incisal distance as follows –

  1. Group A: >35 mm 
  2. Group B: Between 30 and 35 mm 
  3. Group C: Between 20 and 30 mm 
  4. Group D

How is OSMF treated?

  • The most important aspect of treatment of OSMF includes cessation of consumption of arecanut, tobacco, spicy food.
  • Anti-oxidants with vitamin supplements.
  • Intra-oral injections twice a week for minimum period of 2 months.
  • Regular follow-up is very important in such cases. 

Depending on the grade of the condition, the final result of the treatment depends.

Is OSMF a form of cancer?

No. OSMF is a pre-cancerous condition, which means that it has a capacity to develop into cancer if left untreated. To achieve best results, the condition need to be treated early.

Milk teeth are anyways going to fall…

This is the statement most parents make when they have a fussy kid in their house who refuses to brush their teeth. Being a dentist, I come across many such parents with this believe that milk teeth need not be treated. Its like a challenge to explain people the importance of milk teeth. 

But exceptions are always there. This reminds me of a 2-year-old little girl who visited the clinic with multiple decayed teeth. She was anxious, curious but at the same time her family and she herself were very co-operative. That little kid sitting on her mother’s lap, followed all instructions and patiently completed all fillings in her teeth. If a kid can understand the importance of her teeth, then why can’t others?

Milk teeth/baby teeth are 20 in number and every tooth is as important as permanent tooth. Nature has given us everything with a purpose.

There are 20 milk teeth which starts erupting from 6 months of age to about 3 years of age. They need to be maintained till permanent teeth erupts, which starts from 6.7 years of age to about 13-14 years of age (except the third molars – which erupt after about 18 years of age.)

Top 5 reasons to explain why milk teeth are so important –

  1. Space Maintenance – 
    • Milk teeth act as natural space maintainers, till the development and eruption of permanent teeth.
  1. If milk teeth are lost early due to some reason, the tooth alignment is lost and the permanent teeth do not get enough space for eruption at the right place.
  1. This can lead to malocclusion in permanent dentition.
